Documentation of an active online business, Etsy, Artfire or similar account, a trade show contract or a copy of an invoice where you have purchased from another wholesale company.

A copy of a Municipal occupational license from your local area, your Federal Tax ID or EIN, LLC, DBA or Vendor permit.

NOTE: If your business is in the State of Florida, a copy of a Tax ID must be provided also. STATE OF FLORIDA COMPTROLLER SALES TAX DIVISION RULE 39
